Closed vs Open Rhinoplasty
Closed Rhinoplasty
In a closed rhinoplasty, all incisions are made inside the nasal passages. Through these internal access points, the surgeon works beneath the nasal soft-tissue envelope to modify bone and cartilage. This approach avoids external incisions and may be suitable for selected cases where adequate visualization and access can be achieved endonasally.
Open Rhinoplasty
In an open rhinoplasty, a small incision is made across the columella, allowing the nasal skin to be gently elevated. This provides direct visualization of the underlying structures, which can be advantageous in complex cases, revision surgery, or when precise structural modifications are required. Although this technique involves an external incision, the resulting scar is typically minimal and becomes inconspicuous over time.

Ultrasonic Rhinoplasty
Ultrasonic rhinoplasty, Dr Ranieri’s specialism, follows an “open” approach, using a very small incision that becomes barely noticeable within a month. During the surgery, Dr Ranieri carefully reshapes the nose using specialised instruments that vibrate at ultra-high frequencies, allowing precise sculpting and filing of the bone. This technique significantly reduces potential traumas and inflammations associated with other surgical models of rhinoplasty.

Most patients can go home on the same day with a tailored post-operative care plan. Your nose will be supported with a splint for 5 to 7 days to ensure the newly shaped nasal structures stabilise correctly, followed by a check-up to ensure proper healing.
Importantly, traditional nasal packing, which can be uncomfortable to remove, is no longer routinely used. Instead, when internal nasal support is required, soft silicone splints are used. These allow comfortable breathing and are painless to remove. In many cases, internal splints are not necessary at all, making the recovery process even more comfortable.
Swelling generally subsides within approximately two weeks, however it can take up to 12 months for the final results of the surgery begin to become visible. However, with ultrasonic rhinoplasty, recovery is much quicker, between 6 to 8 months.

Every patient has a unique nasal anatomy, skin type, and tissue response; therefore, recovery times may vary. As a general guideline, swelling and bruising are most noticeable during the first 7–10 days, during which a nasal splint is usually worn. After approximately 2–3 weeks, swelling decreases significantly, and most patients are able to return to work and resume their normal daily activities. Over the following 3–6 months, the nose continues to refine as the tissues gradually settle. In general, after about one year, residual swelling has largely resolved and the final result becomes apparent. In cases where ultrasonic rhinoplasty has been performed, visible refinement may begin to appear at an earlier stage.
