Blepharoplasty, commonly known as an eyelift,  is a highly effective solution for rejuvenating the eye area by removing excess skin, reducing puffiness and restoring a fresher, more youthful appearance.  Beyond achieving the desired aesthetic goal, this type of cosmetic surgery procedure can also address sagging skin or eyelids affecting your field of vision. 
 
The result is not only a more youthful and alert appearance, but in many cases, improved comfort and vision as well.

Your options for eyelid rejuvenation

There are three main types of eyelift: lower, upper and complete. The lower blepharoplasty is the technique used to correct slack skin, wrinkles and eyebags, the upper blepharoplasty is focused on improving the look of the upper eyelid, removing excess skin and fat that can create a hooded appearance. The result is a more rested, rejuvenated look. The third option is the complete blepharoplasty, which can be used to correct issues associated with the upper and lower eyelids, and also to achieve the desired aesthetic outcome.

⁨⁨Upper Blepharoplasty

Involves an elliptical incision made along the upper eyelid, from the inner to the outer corner, carefully placed so that the final scar falls within the natural eyelid crease when the eye is open. The predetermined amount of excess skin, and if necessary excess fat is removed. The incision is then closed with very fine sutures.

Upper Blepharoplasty and browlift

In some patients, upper blepharoplasty alone is not enough to correct the “heavy” appearance of the eyelids. This happens when, in addition to excess eyelid skin, there is also brow ptosis (a drooping eyebrow).

In these cases, it is advisable to combine upper blepharoplasty with a brow lift. The brow lift restores the eyebrow to its proper position, opening up the eyes and enhancing the harmony of the forehead and upper eyelid area.

Combining the two procedures provides a more complete and natural result, avoiding the “still heavy eyelid” effect that may occur if only the excess skin is removed.

⁨⁨Lower Blepharoplasty

Is a similar procedure, although the incisions are different and less arched. In this case, the skin is gently separated from the underlying tissues; the appropriate amount of excess fat (fat pads) is removed and, if needed, excess skin is also excised. The skin incision is closed with very fine stitches and sometimes reinforced with adhesive skin closure strips.

The resulting scars are minimal and located just below the lower lash line. In this case we are talking about subciliary blepharoplasty. In some cases when we don’t have skin excess we can use a different technique (transconjuntival blepharoplasty).

Transconjunctival lower blepharoplasty is a modern, minimally invasive technique designed to correct under-eye bags without leaving any visible external scars.

The incision is made inside the lower eyelid, at the level of the conjunctiva, which allows the surgeon to remove or reposition excess fat (fat pads) responsible for puffiness and a tired appearance. 

Because no external skin incision is required, this approach leaves no visible scars and significantly reduces the risk of eyelid retraction or changes in eye shape. Skin is generally not removed, making the procedure especially suitable for younger patients or those with good skin elasticity. Sometimes is possible to remove a little excess of skin. In this case we call it “pinch” blepharoplasty. Pinch blepharoplasty is a simple variation of lower eyelid surgery: it consists of ‘pinching’ and removing a thin strip of excess skin just below the eyelashes, without dissecting deeper tissues or touching the fat pads.

In practice, it is used to tighten loose or wrinkled skin of the lower eyelid in patients with good muscle tone and without prominent under-eye bags.

In some cases, simply removing the under-eye fat pads is not enough to rejuvenate the appearance. For this reason, lower blepharoplasty can be combined with lipofilling (fat transfer).

With this technique, a small amount of fat is taken from another area of the body (such as the abdomen or thighs), carefully purified, and then re-injected into areas of the face that have lost volume, particularly the lower eyelid, tear trough, and midface.
 
The goal is not only to remove the bags, but also to restore facial harmony, soften a hollow or tired look, and improve the transition between the eyelid and the cheek, achieving a more natural and long-lasting result.

Surgery

Depending upon the technique, surgery can last between 1- 3 hours. Type of anaesthesia: Local anaesthesia (superior blefaroplasty) and sedation for the complete or lower blepharoplasty.

Post-operative care plan

Most patients are able to return home the same day with a personalized post-operative care plan. Suture are typically removed in around 5 to 7 days, followed by follow-up appointments to monitor the healing process.

Recovery

Bruising usually fades within about two weeks, after which you will start to see the final results.

In general, the results typically last 5 to 10 years.  Every patient is unique and the results will depend upon factors such as aging, genetics and aftercare. Although blepharoplasty can’t stop the natural aging process, it significantly delays visible signs of tired or sagging eyelids, maintaining a youthful look for years.

Ideal candidates for this type of cosmetic surgery procedure are patients with:

  • Drooping upper eyelids
  • Bags under the eyes
  • Excess skin interfering with vision or appearance

You should also be in good overall health with no serious eye conditions and realistic expectations about the outcome.

As with any surgical procedure, there are risks. To ensure the best results it is essential to use a surgeon who specialises in this field and to ensure you are in good health prior to surgery. Potential complications include:

  • Infection or bleeding
  • Visible scars
  • Temporary difficulty closing the eyes
  • Dry eyes
  • Altered eyelid shape


Fortunately, these issues are rare and generally resolve with proper medical care.

Recovery usually takes 1 to 2 weeks. During this period, swelling, bruising and a degree of mild discomfort are normal. Most of the swelling will disappear within 7–10 days, allowing patients to resume normal daily activities after that time.
